Choice Transfer Request: If your student will be attending school outside of their resident district, please complete this form. All Choice Transfers are processed online through OSPI's Choice Transfer Portal. Follow the link to submit your request. Requests must be filed annually.

Intent to Home School : If your student will be receiving home based instruction, please complete this form. Completed forms should be returned to the White Salmon Valley District Office. This must be filed annually.
Race and Ethnicity Collection: Washington State schools are required to report student data by ethnicity and race categories to the state’s Office of Superintendent of Public Instruction (OSPI). Ethnicity and race categories used in our district are the same as are used in all Washington school districts. They are set by the federal government, the Washington State Legislature, and OSPI. OSPI is required to report the total number of students in various categories in each school to the federal government, but it does not report individual student data. Recently, the federal government and OSPI changed the reporting categories for student ethnic and race data. As a result all student ethnicity and race codes must be updated by the beginning of the 2022-2023 school year. Completed forms should be returned to either your student's school office or to the White Salmon Valley District Office.
